Prospective Providers

How to become a provider? 

Becoming a Sun Health & Dental provider is easy! Visit our website, click the Providers tab, and find “Become a Provider.” Fill out your practices’ information, and someone from our providers department will send you an official contract, and call you to discuss any concerns or questions you may have. 

What do you pay? 

We negotiate a contracted rate with your provider office. This way both your practice, and our members will benefit. With our extensive network of over 15,000 members; becoming a provider of Sun Health & Dental is sure to help your practice grow. All payments are made by members at the time of service. The costs of their appointments will be discussed with them prior to their appointment, assuring for less confusion, and easy check-in/ check-out processes. 


What area are you in?

We are headquartered in Miami, Florida. Our provider offices are located all over South Florida as well as Texas. 


Is this Medicaid? 

No. Sun Health & Dental is a private medical discount plan.


How many patients do you have? 

Our network has over 15,000 members.


Is this an HMO/PPO?

We are a DMPO. A DMPO is a discount medical plan.


What is the age range?

We have members of all ages.

In Network Providers


Where can I get member eligibility? 

Visit our website, click the Providers tab, and find the “Verify Coverage.” There you just input the member I.D. and can immediately check members eligibility.


Do members have an ID card? 

Yes. All of our members receive a physical member I.D. card about 5-7 days after inscription. If a member attends an appointment before obtaining their member I.D, they have access to an online version.


What is the fee schedule? How can I get the fee schedule? 

The fee schedule is the cost of services that our members have exclusive access to. We share our fee schedule with your practice so that when our members attend an appointment, you have all the information necessary to charge them correctly. If at any time you need to view our fee schedule, we can send you another copy.


What lab can we use? Can we use Quest or Labcorp? 

Our network has many labs that our members can attend. Once our members attend their appointment, and receive lab orders, they can quickly call their customer service team at (888) 577-8670 to help schedule their lab appointments.


How can I add a new provider or location?

Existing providers can add a new location or provider to their practice by calling our Provider Department at (855) 941-0240.
